Additional Plan Details
Short-Term Disability (STD)
- 24/7 coverage for on and off the job disabilities resulting from illness or injury.
- Smart 171 Members can choose between 3 flat rate options. Coverage is not offset by other benefits until 80% of pre-disability earnings are attained.
- Benefits are paid tax free
- Pre-existing conditions are covered after 12 months on the plan.
Long-Term Disability (LTD)
- 24/7 coverage for on and off the job disabilities resulting from illness and injury.
- Benefits are paid tax free.
- Pre-existing conditions are covered after 24 months or 12 months treatment free.
- Pays for 5 years after a 268 day elimination period.
- Coverage is not offset reduced by other benefits until 70% of pre-disability earnings are attained.
Life Insurance with Accidental Death & Dismemberment (AD&D)
- Guaranteed Approved.
- New members can enroll in up to $50,000 in coverage and spouses up to $25,000 in coverage and dependents under 26 years of age a flat $15,000.
- Coverage includes an equal amount of Accidental Death and Dismemberment.
- Pre-existing conditions are covered day 1.
- Life coverage is convertible & portable.
